Preliminary results have demonstrated that superior performance can be achieved with the two proposed automated MUNE methods: global path search and alternation condition check. In normal subjects, the calculated modal value and credible interval for the MUNE in hand muscles was 85 (70–108) and a smaller value obtained from the peroneal nerve — mean 48 (40–58).
Metrics of negative-peak amplitude or negative-peak area are less affected by phase cancellation, but the position of peak negativity within individual SMUP waveforms represents phase cancellation and can affect arithmetic addition in determining the average SMUP.37, Normal muscles are innervated by different numbers of motor units, ranging from 100 to 600 motor units.32 A sample of 10 to 20 SMUPs (less than 10 percent of the normal population) is sufficient, with little change in the calculated MUNE values when more than 15 SMUPs are used to determine the SMUP value.38,39. Recording electrodes are placed in a belly–tendon arrangement, and the nerve is activated electrically by percutaneous stimulation to achieve the maximal CMAP. An automated MUNE system could facilitate the accessibility of the MUNE in routine clinical practice if it can provide accurate, consistent, repeatable, and clinically relevant information on motor units. This contention is supported by studies which show delayed conversion to clinical ALS during a prolonged follow-up period in a proportion of subjects initially diagnosed with PLS (Gordon et al., 2006). If the alternation is not detected, the difference of surface potentials could be mistakenly considered as from a motor unit, leading to an over-estimation of the true motor unit number.
